A Molduga “swims beneath the desert’s sand” and, “if it senses sound, it will breach the surface to feast on whatever it can grab” (the game’s description). There are four, all in the Gerudo Desert. All four come from the game’s placement data (zeldamods’ radar for Breath of the Wild), and Zelda Dungeon also counts four. HP is 1,500 in the game’s parameter files, the same as Game8 and Zelda Dungeon. The Molduking is a separate, stronger enemy (1,600 HP in the parameter files) and not one of the four.
How do you beat a Molduga?
Game8 also mentions shooting its belly with an arrow as it leaps, which stuns it too, but calls bombs the more efficient way. Wear your best armor: a Molduga can take you out in one hit.

Beat all four Moldugas at least once, then talk to Kilton at the Fang and Bone (Zelda Dungeon). It sits with the Hinox and Talus medals.
